Flagstaff Bone & Joint will offer sports physicals for athletes Saturday, June 3, 2017, from 8:00 to 9:00 a.m. for last names A – K and 9:00 to 10:00 a.m. for last names L – Z at 77 West Forest Avenue, Suite 301, located in the Physicians & Surgeons Offices attached to the north side of Flagstaff Medical Center.
Each physical is performed by a licensed healthcare/orthopaedic provider and costs $10 (cash only). All proceeds will be donated to the athlete's athletic program.
Athletes should visit http://aiaonline.org/about/forms to fill out forms 15.7A and 15.7B prior to arriving.
