Flagstaff Bone & Joint will offer sports physicals for athletes on:
Saturday, June 1, 2019
8:00 to 9:30 a.m. for last names beginning with A – K
9:30 to 11:00 a.m. for last names beginning with L – Z
Sports physicals will be performed at 77 West Forest Avenue, Suite 301, located on the north side of Flagstaff Medical Center in the NorthStar Medical Offices.
Each physical is performed by a licensed healthcare/orthopaedic provider and costs $10 (cash only). All proceeds will be donated to the athlete’s athletic program.
Prior to arriving, athletes should fill out forms 15.7-A and 15.7-B. To access these forms, please use the button below.
