Financial Sponsorship Request
Written Statement Can Be Completed by the Participant or a Parent/Guardian
Please write a statement expressing your interest in enrolling in Flagstaff Sports Institute’s Injury Prevention and Performance Enhancement Program (IPPEP). This statement can be handwritten or typed. Completed need-based sponsorship requests can be emailed to IPPEP@flagstaffsportsinstitute.com or mailed to 3405 N. Quintana Dr., Flagstaff, Arizona 86001, attention: Flagstaff Sports Institute.
Please be sure to include the following:
- Your athletic or personal fitness goals and current athletic endeavors
- How this program may help you reach those goals
- A personal statement expressing financial need and the amount of financial assistance requested

Please note: If offered financial assistance to complete the IPPEP, participant must be able to attend at least 80% of the training sessions. If attendance falls below 80%, the financial sponsorship to complete the program will be revoked.
This program is designed to decrease the risk of injury and increase measures of sports performance through repetition and consistency in training. Lack of attendance in training will likely affect the positive outcomes of completing this program.
